Job Description

General Summary

Illinois Registered Pharmacist who provides pharmaceutical service to the Institute.  Responsible for compounding and dispensing of medication and prescriptions by Standard procedures and filling prescription requests.  Responsible for all medications and substances leaving the pharmacy.  Assists the Pharmacy Director in planning, organizing, implementing, directing and controlling activities of the department.  Supervises pharmacy technicians.

Principal Responsibilities

  1. Maintains and evaluates medication profile for each patient in the Institute.
  2. Responsible for reviewing prescription orders for correct dosing and to determine that potential drug interactions or allergies will not result from a prescribed substance.  Contacts physician to advise him of potential problems or order clarification and documents results.  Services are appropriate for the age and development of the patient.
  3. Consults/advises with Medical Staff and Nursing Staff concerning information on medications.
  4. Responsible for all medication/supplies or prescriptions leaving the pharmacy for accuracy in filling/refilling medication orders, cassettes or prescriptions with the correct medications, strength, dosage, form, etc.
  5. Assists and supports the Pharmacy Director in planning, organizing, implementing, directing and controlling the pharmacy activities to conform to the Standard Practice of Care and actively promote and participate in improving the Department's Services.
  6. Responsible for assisting the Pharmacy Director in maintaining purchasing and inventory control.
  7. Responsible for supervising/checking daily activities of the pharmacists and technicians including medication order filling, stock supplies, I.V. admixtures, prepackaging of medication, filling of prescription, etc.
  8. Responsible for complete and accurate daily record keeping of controlled substances used in the Institute including monitoring, dispensing and appropriate storage in the pharmacy and nursing units.
  9. Responsible for checking for expiration dates of Emergency boxes and Emergency CPR Carts and replacing of items as needed.
  10. Responsible for correctly filling medication requests for patients leaving on a medical pass or being discharged or outpatients.
  11. Responsible for supervising the preparation of Intravenous Admixtures with special consideration to Aseptic Technique, drug stability, incompatibility, dosage, route of administration, rate of administration and any necessary precautions.
  12. Responsible for checking patient medication cassettes for accuracy before delivery to the Nursing Units.
  13. Responsible for the checking of inventory of the Pharmacy stock and assures use of stack before expiration date.
  14. Responsible for determining patient charging for special or unusual prescription services, pass med, outpatient meds or discharge meds in accordance with established schedules.  Accepts payments for cash transactions and supplies receipt.
  15. Responsible for logging daily charges from Northwestern Memorial Hospital received during the hours the Pharmacy Department is closed.
  16. Responsible for maintaining the Pharmacy Department in a neat and orderly manner.
  17. Responsible for dispensing and packaging of floor stock medications for Nursing Units and other departments.
  18. Responsible for performing Nursing Unit Monthly inspections to resolve any problems that arise and ensure:

a)       All medications are within expiration date and not deteriorated.

b)       Storage requirements are properly addressed.

c)       Only approved medications are present.

  1. Participates in medical health care team rounds, monitors the medication usage of the patients on that service and makes appropriate recommendations.
  2. Provides drug information services to other health care professionals including pharmacokinetic monitoring of target drugs.
  3. Perform all other duties that may be assigned in the best interest of SRAlab.

Other Responsibilities

  1. Responsible for performing other duties as requested/assigned by the Pharmacy Director.
  2. Responsible for preparing, delivering and exchanging patient medication cassettes for the Nursing Units in the absence of technician.
  3. Responsible for providing departmental in-services as necessary.

Reporting Relationships

1.     Reports to the Director, Pharmacy

Job Requirements

  1. A PharmD or a bachelor's degree in pharmacy from an accredited school of pharmacy.
  2. Requires current licensure as a Pharmacist in the State of Illinois.
  3. Work requires approximately six months on-the-job experience in order to gain understanding of Institute practices for the safe preparation and administration of medication and drugs to Institute patients.
  4. Requires analytical skills necessary to determine drug appropriateness and evaluate drug interactions.
  5. Prior hospital pharmacy experience strongly preferred.
